- HFW is a family driven, strengths-based care coordination process that embodies the SOC values and principles at the service level for children and families facing mental health challenges.
SOC Expansion and Sustainability Grant 2016^
- 4-year grant funded by SAMHSA.
- 42 localities funded to provide HFW
- 14 localities funded (localities from 2012 grant) to provide Family Support Partner Services as either a stand-alone service or in addition to other community-based services for youth and families.
